The confirmed programme
- Programme provider
- S.D. Medical Spa, 117 Cross Ave B6, Oakville, ON L6J 2W7. 905-338-7551.
- Monthly contribution
- $99 CAD per month, billed on a recurring basis.
- Account credit
- Your full $99 monthly contribution is added 1:1 to your S.D. account and applied to eligible services and products under the programme.
- Member pricing
- Member pricing applies to all services. Contact the clinic for specific amounts, product benefits and combination conditions.
- Prescription-related treatments
- Included in member pricing. Prescription-related treatments still require assessment and authorization by a Nurse Practitioner.
- Tax
- Prices shown on this site are before tax. HST is added at checkout.
- Billing date
- You choose the date your contribution is billed.
- Credit expiry
- Account credit does not expire.
- Transfer
- Membership can be transferred.
- Pausing
- Membership can be paused. A request can be made once six months have passed since sign-up.
- Additional fees
- There are no additional fees beyond the monthly contribution.
- Care recommendations
- Membership status does not determine clinical recommendations. Treatment suitability and timing are based on individual assessment and professional judgment.

S.D. does not accept paid enrolment through this website. The complete agreement, provided before you enrol, sets out:
- Any minimum term
- Cancellation notice and any cancellation fee
- What happens to unused credit after cancellation
- The exact services and products eligible for member pricing
- Whether advance appointment notification is offered
- Whether membership benefits combine with other offers
- Handling of a declined payment and how to update a payment method
- Any age restrictions
- How S.D. may change the programme, and the notice provided
- The effective date and version of the agreement
